Myofascial Acoustic Compression Therapy (MyACT)

T-Mode – An innovative and unique treatment concept
The scientific literature has found that the frequency of stimuli has a relevant impact on mechanotransduction, i.e., the transformation of mechanical stimuli into biochemical signals. Certain frequencies can be specifically used for therapeutic purposes. Therefore, adjustment of the timing and frequency of pulses (T-Modes) can enhance the effectiveness of MyACT.
RAMP
With the RAMP mode, there is a gradual increase in frequency over a defined period of time. The initial frequency and the ultimate frequency as well as the time period during which the frequency increases are defined before starting treatment. It is used for dynamic applications to treat tendons, the musculotendinous junction, and the muscle belly.
BURT
The BURST frequency mode refers to a series of successive pulse packages interrupted by a brief pause. The duration of pulses and pauses and the frequency are defined before starting treatment. The target tissue experiences a constant shift between the stimulus and respite from the stimulus.

Basic Principle of Myofascial Acoustic Compression Therapy
A sound approach to treating acute and chronic musculoskeletal pain. Myofascial Acoustic Compression Therapy (MyACT) is widely used in the treatment of acute and chronic pain in muscles, tendons and joints. MyACT describes the use of acoustic waves to target tissue at varying depths to compress and manipulate tissue resulting in a focused and precise deep tissue massage. The results of the mechanical stimulus delivered by MyACT can lead to increased circulation and pain relief– key components in the healing process.
For targeted, non invasive pain relief
Mechanical stimuli affect almost all cellular functions of living tissue such as growth, cell differentiation, cell migration, protein synthesis, physiological apoptosis and tissue necrosis. The acoustic waves generated by the PiezoWave2 converge at a point deep within the soft tissue to produce an intense, extremely short duration compression burst. The focused acoustic compression force is translated to the surrounding tissue like an extremely precise deep tissue massage. Why PiezoWave2T and MyACT are Unique
Other companies providing compression devices to treat musculoskeletal conditions do not utilize sound waves to create the compression. These devices rely on a percussive application to the outer layer of soft tissue with the hope that the resulting pressure will translate through the tissue to the targeted injury. The deeper the targeted structure the higher the impact required to the outer soft tissue layer with diminishing clinical results due to pressure absorption and increased patient discomfort at the point of contact.

Piezo ceramic crystals are arranged on a concave transducer in a mosaic pattern. A high-voltage current briefly excites the piezo crystals, causing them to expand simultaneously a few micrometers along their axis to create a pressure wave. The piezo elements are aligned precisely to the therapy focus. The precise, focused, and nonlinear dissemination of the pulse results in steepening at the point of focus, creating a compression event.
The piezo MyACT system does not require an additional reflector. This accounts for the compactness of the therapy source and the precise, well-defined focal zone. Specifically designed for myofascial treatments
Richard Wolf GmbH / Germany and Elvation has specifically designed piezo therapy sources that provide low energy / low pressure levels for myofascial and muscloskeletal massage treatment. Piezo-ceramic elements are geometrically arranged on a concave surface so that when they are excited simultaneously by a brief, high-voltage pulse, they expand by a few micrometers to generate a low energy pressure pulse. The piezo elements are precisely aligned so that each pressure pulse generated focuses in a specific area. This precise focusing of the pulse creates an acoustic compression event at the point of focus.
The PiezoWave2 MyACT system provides you with the choice of two piezo therapy sources for this superficial treatment. The Focused Therapy Source for pin-point, focused treatments and the Linear Therapy Source for larger treatment areas.

Tissue Penetration Depth
The piezo MyACT technology uses interchangeable gel pads which contact the patient to ensure that the acoustic pulse penetrates precisely to the desired depth with as little scattering as possible. These gel pads are used as spacers and change the penetration depth in increments of 5 mm. They accomplish this by drawing the therapy source’s static focal area incrementally superficial or deep respectively. Penetration depths of between 0.5mm and 3 cm are possible, depending on the therapy source.

Mechanism of Action
Acoustic pressure waves are characterized by a pressure surge with an extremely short rise time, which is then followed by an exponential drop in pressure and a brief phase of negative pressure. The pressure waves used in MyACT focus or develop in the target tissue at the point where the pain is originating from. The primary means by which pressure waves affect living tissue is based on the transformation of mechanical stimuli into biochemical or biomolecular signals. Described as mechanotransduction.
Precise pain localization – Patient guided pain relief
MyACT helps define where to treat – a unique feature not offered by other treatment modalities. Diagnosis of referred pain and the recognition of the originating pain triggering points can be accomplished using MyACT. Abnormal musculoskeletal tissue can be “flared” with focused MyACT in order to define the areas that require treatment. This process of defining the origins of pain is guided by the patient through verbal feedback to the healthcare professional providing the treatment.
Steps to locate trigger points or orginating sources of pain:
- Place gel pad directly on the patient’s skin without air pockets and initiate the therapy.
- Start with a low intensity setting of approximately 2-7 and progress based on patient feedback on the intensity of their flared pain.
- Confirm with the patient that the flared pain is familiar.
- Search the triggerpoint by moving the therapy source very slowly and with slight changings of the application angle.
- If no pain can be flared increase the intensity slowly. You may also experiment with the depth of penetration by changing the gel pad if you are unable to localize painful areas.
- The goal is to flare a familiar pain being generated by the trigger point/injury. The constant communication between you and the patient will ensure that you remain on the painful area throughout the intended treatment.
Treatment of Triggerpoints, Enthesopathy and soft tissue injury
The linear focusing therapy source is excellent for treating triggerpoints, Enthesopathies and soft tissue injury – providing a controlled, targeted deep massage –like treatment- capable of delivering compression in deeper muscle regions not reachable with palpation. When moving the therapy source around the treatment area, the patient will feel a deep, dull ache that is familiar to the patient as being like the pain their condition produces. The clinician will ask the patient to report when he or she feels the ache and will adjust the output of the device to the appropriate level tolerated by the patient. The clinician will ask the patient to confirm that the therapy source is still creating the ache and may adjust the location of the treatment based on the patient’s feedback. If at anytime the treatment becomes uncomfortable, the patient is instructed to mention this to the clinician and the output is adjusted to a comfortable level. A typical MyACT treatment takes between 10 and 20 minutes.
Intended Use PiezoWave² MyACT:
The intended use of the PiezoWave² covers the Acoustic Compression Therapy only. Any use of the PiezoWave² outside of the intended use, e.g. for ED therapy, is not approved by FDA and must be considered off-label use. Before using the products, we recommend that you do not solely rely on the information presented and that you always read labels, warnings, directions and other information provided with the products.
